How does Next Wave provide Specialist Support Coordination?

 

When we receive your referral our Client Services Manager will arrange a time to meet with you to discuss:

• Your likes

• Your dislikes

• what  is important to you

• and what you would like to continue or stop doing

 

We will talk with you about your NDIS plan and together decide how to best work toward achieving your NDIS goals and more.

Once our Client Services Manager has met with you, she will find the right person in our team to assist you in achieving the goals you have set out.

It is important that we match you with the right staff member who has experience and empathy in working in the field of expertise that you require.

​

Our Client Services Manager will match you with the right staff member who can coach, mentor, guide, and work alongside you to navigate areas such as:

• Homelessness

• Justice

• Drug and alcohol

• Health needs

• Family issues and needs

• Trauma

• Health and well being

• Financial capabilities

• Education

• Psychology

• Therapies

• Wrap-around supports to assist you to achieve and thrive in the areas that you chose.

We pride ourselves on finding staff that are the right fit for you. Staff who will coach and mentor you, understand your needs, listen to your wants, and stand by you on your life journey.

We will also provide support for you to access non-funded services and create a circle of supports, new activities, and new friends to enhance your life.
